Why Medical Forms Matter
Scouting America requires current medical forms for all Scouts and adult participants to ensure everyone's safety during activities. These forms provide leaders with critical health information, emergency contacts, and consent for treatment if needed.
Important: Medical forms expire after 12 months. Please keep your forms current and provide updated copies to Troop leadership.
Scouting America Medical Form Parts
The Scouting America medical form has three parts (A, B, and C) that are required for different types of activities.
Part A
Required for: All Troop events and activities
Completed by: Parent/guardian
General information including personal details, emergency contacts, insurance information, and medical history. No doctor visit required.
Part B
Required for: All Troop events and activities
Completed by: Parent/guardian (signature required)
Health history including medications, allergies, dietary restrictions, and parental consent for participation. No doctor visit required.
Part C
Required for: Summer camp and high adventure trips (events lasting 72+ hours)
Completed by: Licensed physician, physician assistant, or nurse practitioner
Physical examination signed by a healthcare provider. Schedule your appointment well in advance of summer camp!

Medication Administration
If your Scout requires medication to be administered during Troop events or camp, please coordinate with the Troop medical forms administrator at Troop1128medforms@gmail.com in advance of the event.
Submitting Completed Forms
How to Submit
After completing your medical forms, please submit them to the Troop for our records:
Email to: Troop1128medforms@gmail.com
Format: PDF or clear photo/scan of completed forms
Include: Scout's full name in the email subject line
Important: Forms must be received at least 2 weeks before summer camp or high adventure trips. For regular campouts, submit forms before your Scout's first outing.
Privacy & Security
Medical forms are kept confidential and secure:
- Stored in locked, secure location
- Accessible only to authorized leaders
- Not shared beyond those with need-to-know
- Returned or destroyed when Scout leaves Troop
- Comply with all HIPAA and Scouting America policies
